We are very respectful of your role as a referring provider, and will always honor your relationship with your patient. Indeed, long term care facilities often refer their residents to Hebrew Home or The Hospital at Hebrew Health Care, knowing that it is not our objective to “steal” their patients, but rather to improve their patients’ health to the point that they can be discharged back to the referring facility. Because we operate a large number of services along the older adult continuum of care, Hebrew Health Care offers a one-stop resource for you and for your patients. Regardless of which of our affiliates you first contact, our staff are trained to help you determine which of our services is most appropriate for your patient, and will work with you to ensure their timely admission into the appropriate program.
Our culture is medically driven. We are sensitive to the fact that good medicine requires good communication. Consequently, we provide frequent communications with referring physicians, including discharge summaries and status reports for all referred patients.
