Services: Skilled Nursing
Skilled Nursing | Social Work | Discharge Planning | Dementia | Sub-Acute Rehabilitation
- Our commitment to health care distinguishes us from other long-term skilled nursing facilities. Not only do we have physicians on staff, we have physicians in-house 24 hours a day / 7 days a week.
- Our physicians are board certified and specially trained in geriatrics, with expertise in the health issues that affect older adults. They are complemented by three APRN's who also have expertise in working with older adults.
- Nursing teams comprised of Nurse Managers, registered nurses, licensed practical nurses, and certified nursing assistants coordinate resident care on each of our seven nursing units.
- Our nurses work closely with our physicians, dieticians, physical therapists, occupational therapists, speech therapists, social workers, pharmacists, and recreation therapists to develop, monitor, and continually update care plans.
- Hebrew Home offers on-site dentistry, ophthalmology, podiatry, and x-ray services.
- Our medical teams consult with the area's leading cardiologists, neurologists, psychiatrists, orthopedists, and endocrinologists when our physicians think these specialized services might be necessary.
- The Hebrew Home and The Hospital at Hebrew Health Care for Senior Care are co-located. Therefore, in most cases, residents who need to be treated for exacerbations of symptoms from chronic illnesses can remain in the familiar surroundings of the Hebrew Health Care campus. Of course, if residents need services that The Hospital at Hebrew Health Care doesn't offer, we use our strong relationships with area hospitals to get our residents the services they need.
