Services: Discharge Planning

• In-house assessment of your needs and ordering of medical equipment prior to leaving
• Discharge and medication teaching instructions
• Scheduling of lab work (after the resident has left)
• Assistance with application to Hebrew Health Care affiliate or any other facility if not going home right away
• Assistance with Title XIX applications
